Its Been a While...

Since the last time I posted something useful here
You guess RIGHT, i've been busy for the past several months!
If i told you what i have done for the past several months, you will Never Believe,
especially if you have medical background, haha!

I have been doing something most doctors would not even think of doing !
Never speculate anything you don't know and always have an Open Mind ...

We don't know what we don't know, right?

If God willing, maybe i could share the things later on, together with the amazing results i've been getting. But its not the time yet.

Emergency Department

#ASAP - As Soon As Possible

Hi guys, it been a while since the last time i posted something here.
Although i have many things to share with you guys, like "basic ECG interpretation for houseman so that we don't look dumb" (hahaha). Yes, i may sound crazy but dont get me wrong, this is all based on my own experience, and you can benefit from my experience and be a better doctor in the future. Some of you, i know are medical students, still studying (or busy playing futsal, lol) hard for exams. Exam is exam, i'm not here to replace your 60 years old lecturer, neither am i doing this to benefit from you. In fact i don't have to do this if my initial intention is to get something from the readers, make any sense? So some of the content here, may sound very basic for you but for others, it may not. Just continue reading if what i am going to talk about, does not waste your time, deal?

Currently, our ministry of health, requires you to undergo 2 years of housemanship, consist of 6 postings all together. Read my previous post, if you don't get what i mean.
So i have literally talked about 5 postings, o&g, medical, paeds, ortho, and surgical.Which is why i am going to talk about emergency department to complete the 2 years of housemanship. Cool right? (not really!)
(btw, you can choose between emergency department or anesthesia department for your final posting, you cannot do both)

Why Choose Emergency Department?
Some of you may ask why do i choose ED,
why always ask why? lol
There are many reasons for that, i did NOT just
choose ED after waking up from my wet dream, okay? :D
one of the reason is that, i hate the Operating theater, its too cold (i told you before), other reasons, cant tell you now, hehe..

Basics of Emergency Department
Emergency department (ED), is divided into several zones in order to make the initial treatment more effective and efficient. They divide cases based on severity or the level of emergency of that particular case, which i guess you guys have already know.

So basically we have :-
1) Triage : The very first counter/zone where all cases (i mean all) must pass by. Either the patient is walking, on wheel chair or brought by ambulance, all must be quickly assess by triage. Triage doctor/paramedic will then have to decide where the patient should go next (according to the level of emergency, remember?)

2) Green Zone : This is also known as non-critical zone. Most of the patient is seen here, by the medical officer / houseman. Patient who comes to emergency department, after being assess by triage, and is not critical will be sent here. Understand that this is Emergency Department, so emergency cases will be  given a higher priority! Doctors are human being as well, our ability in handling patients are limited. An average of 400-800 visits per day, depending on which hospital (in 24 hours time!). So green zone patient will have to wait. Even in Green zone, patient is seen according to several criteria, such as baby and older people will be seen first, it is NOT first come first served basis!

3) Yellow Zone : Also called semi-critical zone. Semi laa, not too emergency, not too well also, just nice, haha! Cases in yellow zone is attended on the spot, because semi-critical patient can deteriorate if not given medical attention on the spot. Houseman basically need to take history (from the patient, or family, or ambulance driver or whoever), take vital signs, withdraw blood and much more. Don't worry, because there are plenty of paramedics and nurses to help you, your houseman colleague, at least one medical officer in charge of the zone and ED specialist.

4) Red Zone : This is the critical zone. Where patient can die in the matter of seconds, or maybe already dying! I think the difference between red zone and yellow zone is the equipment. Since government hospital is a "developing hospital" like Malaysia is a developing country, we have limited supply (and fund) to have all the top notch equipment for every bed in yellow zone and red zone. So red zone should have intubation equipment, cardiac monitoring, defibrillator, ventilator and many more that yellow zone do not!

5) Asthma Zone : This is a place for asthma/chronic obstructive airway disease (COAD/COPD), bronchospasm patient, where they need a nebulizer ASAP. Most of them are discharge after several nebulizer given.

Example 1 :
60 years old patient comes with a severe chest pain,
with profuse sweating and look very ill.

Triage main concern is of course heart attack, right? So they will straight send the patient to Red Zone.

p/s : you will learn how to "triage" patient when you're there, paramedic (medical assistant) is your best friend! They are very well trained and is posted there right after they graduate, so learn from them!

Red Zone Criteria
- Always think of the most dangerous part first
- You can always consult your senior if you are not sure, this is not a one-man-show (even Cristiano Ronaldo cannot win the game by himself, lol)

Yellow Zone Criteria
- some criteria may not be here but that does not mean it is not semi-critical!
- always use your intelligence as there are so many disease nowadays! and to make things worst, sometimes they present with atypical presentation (just to confuse us!)

Green Zone Criteria
- Some patient does not even need Emergency Department visit, and thus can be "triage away" to the nearest Klinik Kesihatan.
- This is emergency department for God sake, please educate your friends and family, we should stop talking about politic and educate ourself more!
- if health is wealth, why bother read so much about politic anyway.

Note the G5 criteria!
- STO means to remove sutures, not an emergency, definitely!

A good rule of thumb,
#1 better consult your senior if you are not sure!
(imagine sending a patient to green zone, the patient waited for 4 hours only to find out that he/she is having a silent Myocardial Infarction! some damage is non reversible... careful yeah!

A specialist was once a newbie medical student, who may have failed and repeated his exams.
The Hospital Director was once a cute little baby, crying for mummy at night.
Our first Cardiothoracic Surgeon was once a houseman who failed branula insertion as well.

Never be afraid of failure,
learn from them and improve,
just make sure you don't kill someone in the process, lol

Theres a saying, "no one is too busy in this world, its about priority", i like those kind words, because it shows that updating this blog, is not my top priority, lol

No, I am NOT one of them. Because i'm a walk-the-walk kind of guy, NOT a talk-the-talk kind.
I have target to achieve this year too, most of them are financial target, and currently getting my ass busy for this reason! Have you forgot yours?

Last year I was like most of you guys, i got up from bed, survive another day at work, and go back to bed.
Yes, that was my life cycle, day in and day out. Your life is the hospital, i don't care if you're a houseman, medical officer, specialist, dietician or the security guard. Just have an open mind, and think about it !!

If you hate your current life, list down what is it that you hate about your life,
and start making "resolution" to replace those things, theres no use to post your new year resolution on facebook only to have it "not achieved" by the end of the year !!
And this is why I'm busy, at least for the time being..

to me that is unacceptable, fullstop!

After I started working, during housemanship, other questions started to builds up as well??

is another question that keeps pop-ing, in and out of my thick head,
always feel overworked, underpaid, undervalued, and God knows what.
seeing my friends walked out of housemanship life, i can understand why,
although i survived housemanship, i never feel alive going through it.

Things Happen For A Reason
Just because everyone else is going through the same process, that does not justify
why do we have to work that way, right?
In the first month of my housemanship life itself, i started to look for the answer to my question,
i know i cannot get answer from people around me, as they are doing the same thing everyday.
So i went to the bookstores, going through successful people's biography, just to find the answer to my question.
And ended up reading alot of books, including robert kiyosaki's books !!
And to my surprise, i kind of find out, the ultimate answer to my question.

For your information, i am definitely NOT a book worm to begin with,
In fact, i have never finish reading medicine textbooks from cover to cover !! lol

This is my PROUD collection of books,
which i bought DURING housemanship life
This is not all, still have plenty in my car and at my parent's house. Not to forget, the free reading i've had hours in the bookstores !!

And guest what, yes... I read them from cover to cover !!!  OMG i can't believe reading is so damn cun laa, seriously dude !!
Especially when i read about things i was looking for !!

And believe me, there is so much to learn, i have never thought that "money" is a really interesting subject. I can't tell you how much i have learnt (financially) for the past 2 years. It's like, if i am a pokemon, i have almost max'ed out my level & no other pokemon can kill me, haha !!

Why | Kenapa | Pourquoi
"why" is a very powerful word. In fact, from the 5 wives & 1 Husband ( 5 w = who, where, why, what, when & 1 H = how), i pretty much would say, "why" ranks number one, numero uno baby !!!

Why am i working?
Why i chose to become a doctor in the first place?
Why doctor in Malaysia work like this?
Why do government pays you at the end of month, not at the beginning of month?
Why... Why... the why keeps coming, unless you answer them.

Can't share everything on a single post, will make a kilometers long post if i do so. lol
Long story cut short,
I have found my answer to all my question, well at least almost all.
And it has everything to do with the subject "money", no matter which question i asked !!
So i went looking for books about money, searching about money on the internet and went to countless seminars about money. Spend endless money to learn about money. Again, how irony. Not to brag, but i earn a thousand ringgit more compared to my colleague during housemanship, with passive investment !! Thinking back, this is impossible without me having a strong fundamental about money and the correct mindset at that particular point of time because opportunity like that dont last long, if you hesitate, its gone before you know it !!

p/s : we seldom have control over opportunity, but preparation is 200% under our control. So be prepared all the time, to catch the opportunity, never ever miss the "boat" again !!

What Should You Do

!! With Your Salary !!

Disclaimer : This is what I recommend and what I personally practice. Your creativity is the limit.

Before we start, I will make a BOLD statement here, this is the FUNDAMENTAL guys, I'm gonna say it clearly,

You guys must be thinking, what the heck this guy is talking now, right?
Remember, this blog is about what? - successful housemanship
And this post is about what? - what SHOULD you do with your salary, not what COULD you do !!
Words have different meaning and different powers in it !!

The BIG "why" ??
We have to understand why people do things differently, why people reacts differently and why do people thinks differently towards the SAME things / stimulus.
Its because of the "why" itself. In Cantonese, I mean, its because of "reason". We all have different reason to justify what we think / do / act the way we do. Am I right or right?

So what should I do with my salary?
Before you answer that question, ask yourself first, why do you need the answer to that question?
Just think of a reason, here are some examples :-
- I want to spend it wisely
- I want to optimize the amount of money I can save from my salary
- I want a nice car so that hot chicks will turn their head when i drive pass them

As for me :-
I need to know what should I do with my money so that I can be rich and never have to work again !! (I told you my definition of successful may be different than yours, got it? )
(your creativity or your mind, is the limit, everything is possible and yes it starts from your mind)

So what should you do with your salary, depends on what you want (your "Why" ) to achieve from that salary, there is no absolute answer to that question, am I making sense everybody ??

Now that I have a reason, I know exactly what I want to achieve from my salary, so I can develop a PLAN to achieve that !!

Some of my plan includes :-
  • Make sure I pay the minimum tax and keep my money the maximum
  • Learn the fastest / safest / quickest way to multiply my money (eg : investment)
  • Buy things "I need" instead of things "I want" (eg: car, house, food)
  • Take full control of my money (eg: cashflow, debt)
  • Learn every single thing about money

There's a famous saying;
" If you fail to plan, you plan to fail "
(in other words, if you don't do anything, no plans at all, you will just accept what will happen to you, that is a well-crafted-subconscious "plan" to fail)

So my VERY OWN plan, has to do with :-
- how much money should I save?
- how much money should I invest?
- what investment is the best for me?
- what type of car should I buy?
- what type of house should I buy?

bla... bla... bla... maybe BORING to you, but it exciting for me because this is MY LIFE !!

Computer Vision Syndrome

Oh Mann...!!
Losing My Vision

Since I started this blog, i've been using my computer for longer hours than ever before.
Starring at each and every words i wrote, double checking, just to make sure everything is in place.
When lately, i experience blurring of vision, although its temporary, relief with a good night sleep,
but now headache and neck stiffness starting to kick in. Crap !!

My Eyes Can't Handle It

If you think I simply write things,
Its time for you to think again !!
Lots of research, lots of reading, and of course hours of computer-starring-activity
which directly cause this problem, damn it, helping people can be a real challenge
I did NOT see this coming T_T

Sorry lots more to share, but i'm gonna need some rest,
Yes, this is not a housemanship secret,
This is ME suffering from computer glare overload,
Please don't tell my mom, lol

How To Pay Less Tax LEGALLY

Disclaimer : I am NOT a qualified tax advisor, this is solely based on my research and experience! You should not believe what a total stranger tells you, do your own research & listen to the government, :D

Haven't Heard of "Leverage" Yet !??

Leverage is the ability to do MORE with LESS

To fully understand what I am going to share with you,
You need to read or re-read my previous post, so that you will benefit from it

Again, I am here to share my short experience during housemanship, so my focus will always be and always will, in the point of view AS A HOUSEMAN. However, since we all have a medical degree, please use your creativity, this can be universal once you get the whole idea, if you're a medical officer or specialist (hopefully my boss don't read this, lol), think hard, you be amazed on how much money you've DONATED to the government legally and unnecessarily all these years !!!

To understand this concept, although its not a rocket science, it is just that nobody care to share it with you or medical field related profession just don't care (I bet my money on the latter), you need to master 3 things only !!

1 . How to calculate your Taxable Income
2 . The difference between Relief and Rebate
3 . Simple Primary School Mathematics

Houseman's Income Tax, for 2013
You all did not start housemanship on the same day, so your income can be different from this due to several reason (eg : annual income increment, bonuses, back-dated salary payment, bla bla bla). I have calculated based on a houseman payslip.

So, the total gross income, for year assessment 2013, is RM 65,063.27 (kaya meh this houseman? )
by applying the "tax bracket", the taxable income should look like below :-

** note that the arrow shows gross total income, before and after, minus the relief by LHDN
** the red box shows the tax you need to PAY, before and after, minus the relief

You save almost 50% legally in tax right ?? So what lesson can be learnt here ??
Sure you won't think, because you know i will tell you, right... haizzz...

Le$$on #1 : the more relief you use, the lesser the tax !!
- This is possible because "relief" is design to REDUCE your total income, so less money will end up in the "higher bracket", thus reducing the tax
- In the above case, note that the bracket 19%, is zero on the right side !!

Le$$on #2 : we can aim our income to stop at 7% bracket
- How is this possible? Yes, more or less, houseman income tax looks like this. But the above example, i only use relief which are :-
1) self relief (-9000)
2) EPF (-6000)               ** this total up RM 16,000, note that on the left, money in
3) Books (-1000)                bracket 19% is only RM 15,067.27 , will be nil after relief !!

- To aim for bracket 7%, how much relief do you need, to wipe off the 12% bracket ??
Yes, depends on the money inside bracket 12% lohh !!  Which is about 14k, right ??
- Any more relief you can use? Sure, several of them !! Check LHDN website.

Le$$on #3 : rich people has tons of relief / loophole
- That is why, the rich stays rich and gets richer !!
- They leverage on tax, they have alot of relief given through their company or corporation (not as individual), while they make more profit, they pay less taxes !!
- Make sense or not ??? Now you wonder why they get richer day by day
- Of course this is only one strategy and considered as "small" for them

It's like, either you pay higher tax to the government (of course they will use it wisely)
Use RM 1000 of your money (throughout one whole year) to buy books, get to own the knowledge and income tax relief at the same time !! 

Save your hard-earned money, unless if you love "kangkung" so much !! Mari.. Mari...!!

p/s : some strategy to pay less tax legally, i can't share it here, the government doesn't like it, there is even rules to make sure this "strategy" doesn't circulate to the public. Never mind lah, just pay only, we love our country, it is peaceful, no corruption, we are the best in Asia lorr !!

Why Houseman Need To Know?

  1. Obviously because you are required to pay income tax by law, ignorance is not a valid excuse
  2. You don't want to pay EXTRA on income tax, unless you love to do so
  3. In fact, the Govt already take your hard earned money, to pay income tax, before you even get your hands on those money. (refer to HO payslip screenshot, in my Previous Post, if you fail to notice that)
Take Control of Your Hard-Earned Money
I just can't understand why some doctors and nurses, don't bother to LEARN about Income Tax, let alone to learn about money. I have spoken to a nurse, who still has very little idea when it comes to income tax, although she has been working for the past 25 years, on top of that, her husband retired 5 years ago without any "pencen" and has 2 teenagers studying in local university. I pity her, for her financial constrain, but she CHOOSE to be like that, there's very little room for me to help her.

Self Assessment
This is the system we're using right now, meaning, we (taxpayers) are required to assess ourself (financially), fill + declare our own income per calendar year, calculate the taxable income ourself, make payment (or claim our money back) on time and the best part is, make it right for the first time !!

Who Need To Declare Income Tax ?

Answer :  Anyone who earns RM 26, 501 per calendar year, after EPF/KWSP deduction. Around RM 2300 per month of salary. Yes, we are required by law to declare our income. Other source, give a different amount, but the point is, Houseman NEED to !!

How | Macam Mana
There are 2 ways to declare your income. Offline (using forms) and Online (e-filing LHDN).
My advice, declare using e-filing. Technology is there to make our life easier. Due to the nature of our work, using computer is better, instead of filling up forms and then submit via post or by hand to LHDN.
For first timer, you need to register at LHDN (just need your I/C), after registration, you can fill the e-form later using e-filing at the comfort of your king-size bed !! 

When | Bila
e-filing can only be access 1st March onwards.
Due for submission is April 30th for each calender year.
So this march, 1.3.2014 you will need to declare your income for year 2013 (year assessment).

What If I Don't
Theres Audit from LHDN, they will detect, sooner or later, they will find those who don't declare or under-declare.. but... if you over-declare, they keep their mouth shut !!
Remember, audit is done regularly and extensively, as i mentioned earlier, ignorance is not an excuse. Be prepare for penalty up to RM 20,000 , 3 years of jail or both, if found guilty, and not to forget, you still have to pay the amount of tax you did not declare previously, with interest of course !!

What Happens After I Declare ?
  1. Notice (eg: Penalty if late, Request for more information/receipt)
  2. Repayment (you receive back the money they auto-deduct)
  3. Nothing (govt will spend your money WISELY)

How Much Do Houseman Pay ?
To calculate how much you need to pay (taxable or chargeable income), you need to know the formula. As shown below :-

Taxable Income = Total Income - Tax Relief
After you get your taxable income, there's a way of calculating it, using "income bracket"
Refer HERE, on how to calculate, because someone else has done a good job on explaining this, and i'm lazy btw, lol. Do check with LHDN website, as these bracket can change anytime the govt want.

Know The Difference Between "Relief" and "Rebate"
Relief is use to reduce your income, so that you'll end up in a LOWER bracket, thus lower percentage of tax. Meanwhile, rebate is the amount of tax money that you can CLAIM BACK (since they auto-deduct it).

Example :-
At the end of year assessment 2013, our beloved govt has auto-deduct my income, total of RM 500. After i calculated my taxable income, the total income tax (after i deduct all the relief) i should be paying is RM 3400. My maths is telling me that, i need to pay the govt another RM 2900 ( 3400 - 500 ) right?

But wait a minute, i paid RM 5000 of Zakat in total that year. 
As any zakat is considered as REBATE, regardless of the amount, so now, i DON'T have to pay the RM 3400 (total of tax according to my bracket), which means, i DON'T have to pay the "remaining" RM 2900 and the govt has to REPAY me back the RM 500 they auto-deduct initially.
Note that i get RM 500 of "rebate from the govt" only, NOT the total amount of my paid zakat !!
